Terms and Conditions

Consent for Participation

I hereby agree to participate (or allow my child/ward to participate) in pottery and painting classes provided by Jey Art Studio (“the Studio”). I understand that these classes involve hands-on activities, including but not limited to working with art materials, tools, and equipment that may involve certain risks. By agreeing below, I confirm that I have received all necessary information about the program and voluntarily consent to participate (or to allow my child/ward to participate) in these activities.

Acknowledgment of Risks

I acknowledge and understand that participation in pottery and painting classes may involve risks, including but not limited to:

• Minor cuts, scrapes, or burns from handling pottery tools, clay, or other materials.
• Allergic reactions to paints, glazes, or other art supplies.
• Accidental injuries due to slips, falls, or misuse of equipment.
• Staining or smudging of clothes due to working with paints, glazes, clay, or other art materials.

I acknowledge that the Studio has taken reasonable steps to minimize risks by providing instruction, supervision, and safety measures. However, I understand that it is impossible to eliminate all risks entirely.

Waiver of Liability

In consideration of the opportunity to participate in these activities, I, on behalf of myself (and/or my child/ward), hereby release, waive, and discharge Jey Art Studio, its owners, employees, instructors, and agents from any and all liability, claims, demands, or causes of action, including personal injury, property damage, or loss arising from participation in the classes, except where caused by gross negligence or willful misconduct.

Medical Consent

In the event of a medical emergency, I authorize the Studio to seek medical assistance for myself (or my child/ward) if deemed necessary. I understand that I am responsible for any costs incurred as a result of such medical treatment.

Photo/Video Release

If you agree to allow Jey Art Studio to use your photo on our website, newsletters, or social media, please grant permission verbally or in writing.

Newsletter and Updates Consent

If you agree to receive newsletters, updates, and information about upcoming classes, events, and programs from Jey Art Studio, please grant permission verbally or in writing.

Agreement and Signature

By agreeing below, I confirm that I have read and understood the terms of this Consent and Waiver of Liability Agreement and agree to the terms outlined above.
